At the surgery

Use the tear-off slip of your computerised prescription and bring it to the surgery, or put it in the box on the reception desk. We can post it to you, but we do ask for a stamped address envelope

By fax

You may fax your request on 541001 for Park Farm patients, 298977 for Vernon Street patients

On the web

By using the repeat prescription request form on this page (you must be registered for our online services to do this). This facility will allow you to request a repeat prescription of any of your medications which are currently authorised to be issued. If your review date has passed the medication will not be able to be requested by this method. You may already have had a request from the practice to attend for a medication review
